your problem is from movement in your tiles. Something is causing them to move .... i never installed a floor with the method you mentioned of installing a sub-base for the tile ... I use wonderboard cementious tile backer board of no less than 1/2 " on all wood subfloors. 1/4" isn't enough to prevent movement in my opinion, but i am not aware of the products and procedures you used.
